" I am excited to be a Millennium Fellow because it offers a unique opportunity to interact and learn from a diverse group of like minded individuals. It also provides a platform and the tools to realise my goal which is to raise the level of healthcare in my community. I look forward to working with the other Fellows as they will also motivate me to become a better leader. In the words of Jiddu It is no measure of health to be well adjusted to a profoundly sick society and we have to ensure that our society is by all means not sick. "

Millennium Fellowship Project: The Aftercare Initiative

The Aftercare Project aims is targeted at individuals in the community with Noncommunicable diseases such as diabetes, kidney disease, hypertension and seeks to improve the follow-up management after they have been discharged from hospital as many such patients in my country are not able to access this care. The Aftercare Project seeks to create a database on which patients with various conditions may register their contact details according to their conditions and and which is then used to schedule appointments and regularly contact patients for follow up on their conditions.
This database will seek to register 20 new patients weekly from a local hospital and in the course of 3 months, accumulate a register of upto 250 individuals who can be helped with the follow up and management of their conditions.
