Millennium Fellowship Project: Sustainable Healthcare Solutions for Older Persons
The project named Sustainable Healthcare Solutions for Older Persons follows the Community Geriatric Care Model. My project increases access to NCD health care services among older persons in rural areas. To achieve this, youthful community
members (volunteers) were trained on basic nursing skills and how to use digital blood pressure machines and glucometers. They were assigned older persons in their community who have been diagnosed with Hypertension/Diabetes, they monitor and record their Blood Pressure/ Blood Sugar at least twice a month
and these readings together with a brief history are relayed to a doctor. To refill the medications for NCDs, the trained volunteers meet a doctor at least once a month. The refilled medicines are delivered by volunteers to the older persons without having them move to the health facility. When a doctor needs to see an older person for physical review or further investigations, the volunteers assist the older persons to meet the doctor. Volunteers also help with nursing care for bedridden seniors and spot worsening or burdensome signs and symptoms and report to the doctor promptly.
About the Millennium Fellow
Simon Peter Akopan is a responsible student at the Medical School of Makerere University. He was born in Uganda and has lived in several regions of the country. This made him to appreciate what people go through in his beautiful nation. Simon is passionate about the ensuring that social injustices are dealt with and is particularly involved in ensuring that the older persons live dignified and fulfilled lives in a country that is largely youthful. This has seen him design programs to address non communicable diseases, advocate for the older persons and actively being involved in the drafting of his country's first ever Strategic Plan aimed at addressing the health care needs of older persons. To achieve this, Simon has demonstrated exceptional leadership skills to organize his colleagues to achieve more for the disadvantaged communities.
