Millennium Fellowship Project: Chronic Aid (Fight against DM)

This foundation is dedicated to supporting individuals living with chronic medical conditions, with this year’s focus on diabetes and hypertension. The project aims to expand its services in the coming years to reach more people with chronic illnesses. During the initiative, individuals were identified, scheduled for clinic visits, and provided with a free monthly supply of medications. The project helped improve access to healthcare, ensured continuity of treatment, and promoted better disease management for participants, laying the groundwork for long-term support and health advocacy for people living with chronic conditions.
